Our client, a leading provider of community and social support services, is looking for a compassionate and experienced Senior Case Manager to join our dedicated remote team. In this fully remote role, you will play a vital part in supporting vulnerable individuals and families, coordinating comprehensive care plans, and connecting them with essential resources. Your responsibilities will include assessing client needs, developing individualized support plans, and advocating for clients to ensure they receive appropriate services. You will work collaboratively with a network of community organizations, healthcare providers, and government agencies to facilitate access to housing, employment, mental health services, and other critical support systems. The ideal candidate will possess a strong understanding of social work principles, case management best practices, and the challenges faced by diverse populations. Excellent communication, active listening, and problem-solving skills are essential. You must be empathetic, patient, and highly organized, with the ability to manage a caseload effectively and maintain accurate client records in accordance with confidentiality guidelines. While this role is remote, knowledge of social service landscapes in areas like **Mombasa, Mombasa, KE**, and surrounding communities will be highly beneficial. You will also be responsible for crisis intervention, conducting follow-up assessments, and contributing to program development and evaluation.
