- Maintain accurate and confidential client records in accordance with organizational policies and legal requirements.
- Collaborate with a multidisciplinary team of mental health professionals, social workers, and other service providers.
- Provide guidance and mentorship to junior case managers.
- Facilitate psychoeducational groups and support groups as needed.
- Master's degree in Social Work, Psychology, Counseling, or a related field. Relevant professional licensure (e.g., LCSW, LPC) is highly desirable.
- Minimum of 5 years of experience in case management or direct service provision within the mental health sector.
- In-depth knowledge of mental health conditions, treatment modalities, and relevant legislation.
- Proven experience in developing and implementing care plans.
- Strong understanding of community resources and referral networks.
- Excellent assessment, crisis intervention, and de-escalation skills.
- Exceptional communication, interpersonal, and advocacy skills.
- Proficiency in using electronic health record (EHR) systems and other relevant software.
