Our client, a leading provider of community-based social care services, is seeking a dedicated and compassionate Senior Care Coordinator & Case Manager to join their fully remote team. This role is vital in ensuring that clients receive comprehensive, person-centered support and navigate the complexities of the healthcare and social services systems. You will be responsible for developing care plans, coordinating services, advocating for clients, and managing a caseload with empathy and professionalism. The ideal candidate has extensive experience in case management, a strong understanding of community resources, and exceptional communication and organizational skills necessary to excel in a remote work environment.

- Supervise and mentor junior case managers or care support staff (as applicable).
- Ensure compliance with all ethical standards and professional guidelines.
- Bachelor's degree in Social Work, Psychology, Nursing, Public Health, or a related field. A Master's degree is a plus.
- Minimum of 5 years of experience in case management or care coordination, preferably within community and social care settings.
- In-depth knowledge of local and national social services, healthcare systems, and relevant legislation.
- Strong understanding of client assessment techniques and care planning processes.
- Excellent interpersonal, communication, and active listening skills.
- Proficiency in using case management software and electronic health records.
- Demonstrated ability to manage a complex caseload and prioritize tasks effectively.
- Strong problem-solving and crisis intervention skills.
- Ability to work autonomously and collaboratively within a remote team structure.
- Empathy, compassion, and a strong commitment to client advocacy.
- Relevant professional certifications (e.g., LSW, LCSW, RN) are highly desirable.
