Our client, a compassionate and reputable provider of community services, is seeking a dedicated Community Support Manager to oversee their elderly care programs. This role is based on-site, requiring a strong local presence to effectively manage services and connect with clients. The Community Support Manager will be responsible for coordinating and delivering high-quality care and support services to elderly individuals within the community. This includes managing a team of caregivers, developing personalized care plans, liaising with families and healthcare professionals, and ensuring the well-being and safety of all clients. Responsibilities include:
  • Managing and supervising a team of community caregivers, including recruitment, training, scheduling, and performance evaluation.
  • Developing, implementing, and monitoring individualized care plans tailored to the specific needs of elderly clients.
  • Ensuring the delivery of compassionate, safe, and high-quality personal care, domestic assistance, and companionship services.
  • Acting as a primary point of contact for clients and their families, addressing concerns and providing regular updates.
  • Collaborating with healthcare professionals, social workers, and other service providers to ensure a holistic approach to client care.
  • Conducting regular home visits to assess client needs, evaluate care quality, and ensure client satisfaction.
  • Maintaining accurate and confidential client records, including progress notes, care plans, and incident reports.
  • Ensuring compliance with all relevant health and safety regulations, policies, and procedures.
  • Managing the community care budget and resources effectively.
  • Promoting the organization's services and building positive relationships within the local community.
  • Identifying opportunities for service improvement and program development.
  • Responding to emergencies and providing support as needed.
  • Organizing social activities and engagement opportunities for clients.
We require a Bachelor's degree in Social Work, Nursing, Gerontology, or a related field, along with a minimum of 4 years of experience in social care, community health, or elderly care services, with at least 2 years in a supervisory or management role. Excellent interpersonal, communication, and problem-solving skills are vital. A valid driver's license and access to reliable transportation are necessary for home visits. Knowledge of local community resources and healthcare systems is advantageous. The ideal candidate will possess strong empathy, patience, and a commitment to improving the lives of the elderly.
