Our client, a leading pediatric healthcare provider, is seeking a compassionate and highly organized Senior Pediatric Care Coordinator to lead their remote care management team. This position plays a crucial role in ensuring children receive comprehensive, timely, and coordinated care. You will be responsible for facilitating communication between families, medical professionals, and community resources, advocating for the child's needs, and managing care plans. This fully remote role requires exceptional interpersonal skills, a deep understanding of pediatric healthcare systems, and the ability to navigate complex patient needs with empathy and efficiency. You will be a vital link in the healthcare journey of young patients, ensuring they and their families receive the support and resources necessary for optimal health outcomes.

- Develop, implement, and monitor individualized care plans for pediatric patients.
- Coordinate appointments, referrals, and follow-up care with physicians, specialists, therapists, and other healthcare providers.
- Serve as a primary point of contact for families, providing support, education, and guidance regarding their child's health.
- Liaise with insurance providers to ensure coverage and facilitate access to necessary services.
- Identify and connect families with community resources and support programs.
- Advocate for patients' needs within the healthcare system and ensure adherence to treatment plans.
- Maintain accurate and confidential patient records and documentation.
- Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and educators.
- Conduct regular assessments of patient progress and make adjustments to care plans as needed.
- Mentor and supervise junior care coordinators, providing guidance and support.
Qualifications:
- Bachelor's degree in Nursing, Social Work, Public Health, or a related healthcare field. A Master's degree is a plus.
- Minimum of 5 years of experience in pediatric healthcare, care coordination, case management, or a related role.
- Demonstrated experience working with children and families, particularly those with chronic or complex medical conditions.
- Strong knowledge of pediatric health conditions, treatment modalities, and community resources.
- Excellent communication, interpersonal, and negotiation skills.
- Proficiency in electronic health record (EHR) systems and other healthcare-related software.
- Ability to work independently, manage a caseload effectively, and prioritize tasks in a remote setting.
- Compassionate, patient-centered approach with a strong commitment to advocacy.
- Certification in Case Management (e.g., CCM) is highly desirable.
