- Bachelor's degree in Social Work, Nursing, Gerontology, or a related field. A Master's degree is a plus.
- Minimum of 6 years of experience in social work, nursing, or community care, with a specialization in geriatric services.
- Proven experience in care management, assessment, care planning, and client advocacy.
- Strong understanding of the physical, cognitive, and psychosocial needs of older adults.
- Excellent leadership, communication, interpersonal, and problem-solving skills.
- Ability to manage a remote team effectively and utilize technology for communication and case management.
- Knowledge of relevant legislation, funding streams, and community resources for seniors.
- Proficiency in using care management software and electronic health records (EHR).
- Empathy, patience, and a genuine commitment to improving the lives of seniors.
