Position Summary
To provide assistance to people receiving services throughout all aspects of their lives across educational, residential and community environments. Provides support for activities of daily living (ADL's) including personal care. Promote choice and independence while providing protective oversight. This may include, but is not limited to, attendance at spiritual activities, various recreational activities (such as swimming), and attending family functions for the people or person you support

- Complete required billing documentation for supports and services provided
- Obtain and maintain all applicable training, certifications, and knowledge of policies and procedures
- Ensure safe and clean environments, including grounds and vehicles, for the people we support

Position Summary
In conjunction with the Assistant Director, the SDS Program Coordinator will be responsible for the administration of Self-Directed (SD) Fiscal Intermediary (FI) Services. Duties include general administration and tracking of SD budgets, prompt billing and payment of approved services, Medicaid and corporate compliance adherence and employer or record duties for all "self-hired" staff. Services provided through these plans allows individuals to live, work and grow in the most appropriate, integrated settings designed by them and their Circle of Support (COS) team.
Primary Duties And Responsibilities
- Adherence to "person-first" philosophy, goals set forth by the Department and Springbrook policies and procedures at all times
- Have respectful, person-centered communication with supervisors, individuals, families, staff and community partners (OPWDD, service providers, vendors etc.)
- Meet with assigned individuals, as needed, to coordinate aspects of the service plan within the fiscal intermediary (FI) established level of support
- Audit each SD plan/budget on a regular basis to ensure all appropriate documentation has been received from all responsible parties to avoid any interruption in services
- Understand the SD Budget process including the role of each member of the COS team, budget entry in the on-line system, and roles and permissions within this system
- Maintain, review and submit billing documents and payroll as required by the department of labor, Medicaid and Springbrook on an ongoing, timely, and efficient basis
- Maintain required documentation to ensure Medicaid and corporate compliance standards are being met
- Report all potential corporate compliance and/or possible 633/624 incidents to Supervisor immediately and complete reporting documentation when necessary or instructed to do so
- Assist with New Employee Orientation and follow up with SD staff to ensure all required training is completed both immediately after hire and on an annual basis
- Reasonable response time to individual requests and concerns, in the most convenient method of communication chosen by the individual, plan designee and/or staff
- Arrange and attend launch meetings (in person) for all individuals on your caseload
- Attend ISP's and COS and planning team meetings either by phone or in person, upon request and availability
- Assist individuals and/or designees with HR related questions and provide policy and guidance as the employer of record
- Monitor overall budgets for payroll and expenditure issues- inform individuals and support brokers of any compliance issues or potential overages within the budgets
- Assist program Administrative Assistants with contacting Support Brokers, Care Coordinators and others for outstanding documentation related to OMIG, OPWDD and Springbrook required information
- Follow OPWDD SDS Guidance Document as well as HCBS Waiver Service regulations
- Travel to team meetings and to other Springbrook locations as needed
- Emphasize and evaluate satisfaction of those receiving services; tracking grievances and following up on concerns with your direct Supervisor as well as at department meetings

The Community Homes Residential Manager is a creative, flexible, organized, and compassionate leader, who ensures that the home they manage is a place of shelter, safety, and serenity for the residents who chose our program. The Residential Manager trains, guides, and engages with their team of Direct Support Professionals. Under the guidance of the Residential Manger, DSPs in each home are facilitators, mentors, and agents for meaningful choice for the people who chose to make a Springbrook home their home.
Position Summary
The Residential Manager is responsible for the overall function and oversight of their assigned location(s). The Residential Manager's principle duties and responsibilities include, but are not limited to:
Principle Duties And Responsibilities
- The typical schedule is Monday – Friday 8am to 4pm with one (1) weekend day per month. However, Residential Managers may need to change their schedule based on the needs of the program and workload. This may include day, evening, overnight, weekend, and holiday hours.
- Residential Managers will participate on the 24-hour on-call rotation process. This may include responding in person for specified situations resulting in health and safety for residents. Follow on-call guidance.
- The Residential Manager is responsible to hire, train, supervise, engage and retain Assistant Residential Managers and Direct Support Professionals (DSPs).
- Ensure that employees support personal outcomes, safety and security of individuals receiving services.
- Conduct and/or participate in Life Plan meetings, assessments, or other meetings with individuals receiving services and/or other service providers.
- Conduct clinical and administrative reviews of Habilitation Plans, monthly notes, and Medicaid billing to ensure compliance.
- Complete all required documentation reviews to ensure compliance with policies, procedures, laws, etc.
- Ensure compliance with all regulations and P&P as dictated by all regulating agencies and State and Federal laws.
- Responsible for the schedule, time and attendance, PTO requests, learning compliance and any other staff related operations.
- Conduct regular staff meetings, monthly employee meetings, annual reviews, and employee performance reports.
- Ongoing communication and collaboration with family members, advocates, legal guardians, and other treatment team members.
- Follow all 633/624/625 regulations, Incident Reporting, investigation process, IRMA entry, follow up and completion of recommendations as needed.
- Identify, report, and/or correct all safety hazards including physical plant problems including the completion of fire drills (monthly and as needed).
- Audit resident personal allowance funds and follow Springbrook personal allowance policy and procedures
- Maintain medication certification per Springbrook policies

- College degree preferred. May use a combination of education and/or experience that meet the requirements for this position.
- Minimum three (3) years' experience working with individuals with developmental disabilities preferred.

- The Residential Manager should possess good analytical, written and oral communication abilities, interpersonal skills, and effective time management and organizational skills.
